Education and Experience Required:
Licensed Registered Nurse. Maintains a current/unencumbered RN license in the state. Minimum of 1 year of clinical experience in long term care, community based care or assisted living. Quality assurance/improvement and staff development experience. Prior experience and training working with the disabled and elderly population and their families faced with emotional and behavioral challenges.
Minimum Physical Requirements:
Must be able to go up and down stairs; bend and lift; and perform physical restraints.
Position Summary: This position supports Lillie’s Healthcare, LLC community-based residences. The Registered Nurse (RN) will be responsible for maintaining quality of care throughout the facility, promoting the health and wellbeing of each resident, and reporting observed changes in resident condition or health to management team. He or she will also be required to assist with training of direct care workers and management on implementation of facility procedures.
Responsibilities:
- Ensures quality resident care is consistently delivered in accordance with all local, state and federal guidelines.
- Partners with management on the new hire orientation, training and in services for the direct care workers.
- Provide oversight of the medication management program and identify any risks/issues/concerns with program to promote resident safety in the medication use process. Notify management team of any identified risks/issues/concerns.
- Collaborates and partners with management and direct care workers to promote quality care and independence for our residents.
- Perform and document resident admission and monthly resident assessments.
- Maintain, through communication and documentation, positive and collaborative relationships and continuity with other care providers.
- Ensures compliance, efficiency and team’s understanding of policies and procedures.
- Create new policies and update existing policies to help improve the level of care for each patient.
- Ensure that all patient care plans have the documentation necessary to provide the proper type of care.
- Participate in scheduled On-Call duties as needed.
